AlertDriver is an internet-based information and communication service. The purpose is to educate people about the effects of distracted and aggressive driving and to suggest ways to help improve driver performance. Nothing contained in the course is intended to replace qualified driver instruction in a classroom or on a closed course. The information should not be considered complete, nor should it be relied upon as a complete guide to driving instruction. If you believe that you have a medical illness or disorder that contributes to your level of distraction or aggression, you should consult a qualified health care provider.

Preserving your privacy is important. We will ask you for your email address so that we can give you feedback on your responses. If you are a court referral, your personal information is requested to keep track of your progress. All responses on the survey are strictly confidential and are not available to the court, your employer, insurance company or any licensing agency. Your responses are used only for instruction and research purposes. The data about a group or your anonymous comments may be published, but identifying individual information will not be released to anyone.

I understand that I will be asked to provide "reflective chat" answers to a number of questions. Your chat comments will be read by our administrative staff and some will be posted to the course without editing. We will also post your initials, age, gender, and date of completion. There are no right or wrong answers, but if your answers are incomplete, missing or inappropriate, we will not issue a Certificate of Completion. Copy and Pasted answered responses will result in a certificate not being issued.

I understand that Profile Associates and alertdriver.com will make every reasonable effort to have the on-line program available each time I log on. However, due to circumstances beyond their control or scheduled maintenance, I understand that there may be intervals during which the on-line portion of the program will not be accessible
